I understand that the Support For Students Growth Center (S.S.G.C.) strives to help as many clients as they can. However, I understand that not all applicants are accepted for services. I understand that the S.S.G.C will use the application information to assess my child’s needs for the purposes of evaluating their abilities and needs and determining placement and service options. Recommendation and approval of my child for services provided by the S.S.G.C is at the discretion of the S.S.G.C administrative team.
Camp Attendance: Online summer camp relies on camper’s regular attendance for all campers to have the greatest amount of enjoyment and make optimal progress. It affects the whole group whenever a participant is absent, is distracted by outside forces, signs on late or signs off early. Each camper is registered for an individual camp session that meets, online, at a specified time for 1 -hour each day Monday through Friday. Please keep in mind that we are reserving your camper’s spot at camp and are staffing and adjusting our activities and curriculum accordingly, whether he/she is able to make it or not.
Use of Service: Our online camps utilize services provided by a third-party. For this reason, SSGC will not be financially liable for any connection issues which are out of our control. You will need to ensure you have a proper internet connection at all times to participate in our camps. Please check with your internet service provider if you have any connection questions. You may also contact Zoom.us with any question about use of their service.
Refund Policy: All payments and deposits, are non-refundable. Refunds will not be given for withdrawals or dismissals after a session has begun. If a child engages in conduct which is harmful, dangerous, or extremely disruptive to the wellbeing of the “campers” in our online camp or the personnel of S.S.G.C., we reserve the right to dismiss the child, and a prorated refund may be offered, at the discretion of the administrative officers. Refunds or credits will not be issued for any missed camp sessions*.

Information on Behavioral Expectations and Supervision: Every camper is unique in his/her needs and abilities, as our online camp occurs virtually in a location out of the physical control of the S.S.G.C., the S.S.G.C. cannot be liable for any actions taken by or to the camper during camp time. It is the parent’s responsibility to ensure the camper( s) are safe and have the internet connection, computer and behavioral ability to connect to our online camp and follow the established rules. We strongly recommend that parents supervise their campers during camp time to a level the parent or guardian deems appropriate. The S.S.G.C. cannot be held liable to loss of Internet connectivity or technical problems on the part of the camper(s) or counselors.
I agree to the cost and payment terms agreed upon and understand that these services will likely not be paid for or reimbursed by my insurance company. I agree to hold harmless the S.S.G.C as well as its employees or representatives from any damages or losses of any kind including direct, indirect, incidental, consequential or punitive damages arising out of the applicant’s and/or child’s participation in their programs. I understand that the S.S.G.C makes no warranty or guarantee of any kind whatsoever regarding results or outcomes, whether direct or indirect, from the services it provides. In submitting this application, I certify that the information provided herein, including all enclosed documents, is complete and accurate to the best of my knowledge. I understand that failing to provide complete and accurate information in or with this application will void any potential refunds or credits.
